First, athletes and parents must complete all forms on FinalForms at FinalForms.com and get an IHSAA Physical Exam prior to being allowed to practice on the first official day of practice. Physical Forms may be picked up at Scott 2 Central Office at Door 1 from 8 am to 3 pm Monday through Friday.
